    Substance Abuse Counselor

    The Substance Abuse Counselor performs comprehensive assessments, service planning, and service coordination to address multiple aspects of a client’s life, develops client treatment plans based on practice standards, clinical experience, and client histories and educates clients on substance abuse services.

    Position Responsibilities:

    Provide counseling and case management services to participants admitted or seeking admission to the program

    Complete intakes and assessments utilizing the ASAM Criteria which includes assessing across the six dimensions to determine the appropriate level of care

    Facilitate education/treatment groups and individual counseling

    Document all services and maintain thorough and accurate records in accordance with DASA Rule 2060 and A Safe Haven policies

    Complete discharges summaries for clients

    Develop treatment plan goals and objectives with the client in order to provide best individualized treatment options

    Conduct treatment plan reviews within the required timeframe in order to evaluate progress and/or modification of goals

    Develop a continued care plan with the client prior to discharge

    Develop curriculum and lesson plans based on the needs of the client population and review with Supervisor

    Develop client treatment plans based on practice standards, clinical experience, and client histories

    Provide case management services such as coordinating with probation officers, case managers, etc. regarding the needs of the clients and providing referrals based on those needs

    Review and evaluate clients’ progress in relation to goals described in treatment and care plans

    Review all charts in preparation for annual audits

    Coordinate case management with appropriate support staff

    Attend clinical staff meetings

    Maintain knowledge of A Safe Haven’s cultural competencies in order to better serve clients needs as well as the organization as a whole

    Participate in staff meetings

    Participate in trainings associated with job responsibilities

    Qualifications:

    Education/Experience

    Bachelor’s degree (B.A.) from four-year college or university preferred in social work, psychology, or criminal justice related field; or two years of related experience and/or training; or equivalent combination of education and experience

    Experience with male and/or female offender populations a definite plus!
