The SHARE Program is a JCAHO accredited, non-hospital based, 56-bed detox and residential rehab substance abuse treatment facility for adult male and females 18 years and older located in Hoffman Estates. We are seeking a full time counselor for the female residential rehab unit.

Job Duties include:

Assessment of the client’s strengths, weaknesses, problems and needs to assist in the development of a treatment plan

Treatment planning

Case Management

Facilitating lectures, specialty groups, MISA groups and process groups

Addressing client behaviors

Participating in weekly staffing

Participation in daily community meetings

Continuing care planning

Discharge planning

Documentation via the electronic record

Providing all documentation timely necessary for UR staff to communicate with insurance companies for continued stay

Job Requirements include:

Knowledge of Rule 2060, ASAM criteria and treatment planning for residential clients

Previous inpatient experience preferred.

CADC and Bachelor’s Degree necessary. LCPC or LCSW helpful.

One evening shift a week and 3 week weekend rotation

Ability to work cohesively on a multi-disciplinary team

Ability to communicate both verbally and in writing in a clear concise manner

Excellent organizational skills

Self–starter and the ability to work independently and as a team member

Commitment to helping build and maintain client census

Ability and willingness to provide supervision to interns
