An individual’s return to the community from incarceration is a challenging transition. Upon release, people often struggle with substance abuse, lack of education and employment skills, limited housing options, and mental health issues. In an effort to expedite access to the wide array of services and supports needed to help individuals successfully integrate into the community, the Reentry Case Manager will be an integrated clinical/employment/reentry-case management related role within Safer Foundation. The Reentry Case Manager will possess the expertise and clinical skills needed to address the complex needs clients face. Housed under the Safer Treatment Center, but working in an integrated/cross-functional manner with many components of the organization, the Reentry Case Manager will work to triage the many needs and issues individuals face when coming to Safer for employment services.
Specifically, Reentry Case Managers will assess an individual’s ability to work and job readiness, including their psycho-social needs that may affect their employability and overall sustainability to live successfully in the community. Reentry Case Managers will determine the best fitting-pathway/program for the individual to obtain and sustain meaningful employment. In instances where individual needs additional supports and stabilization, the Reentry Case Manager will continue his or her relationship with the individual by 1) providing integrated employment and clinical counseling/case-management services directly, 2) connecting him/her to other programs within Safer, and/or 3) linking clients to external providers in the community.
Essential Duties and Responsibilities:
Receive direct and indirect referrals to Safer
Complete integrated intake assessments, including demographics, housing status, insurance information, employability, physical and behavioral health screens, and if needed, in-depth behavioral health assessments and or linkages to address immediate, urgent needs.
For individuals with immediate/urgent needs, identify the appropriate internal and external providers/services and facilitate access by coordinating/setting up appointments, pre-requisite information, verifying benefit coverage, arranging transportation, coordinating with family/friends, and parole, etc.
As a result of the intake process, determine best career pathway forward with client.
For individuals in need of additional supports, develop a treatment/service plan with the client that addresses short term and long-term goals in relation to healthcare, housing, social, and employment related goals.
Directly or working with another employment specialist or therapist, conduct a variety of group sessions focused on job readiness, life skills, drug education, health and wellness, and other topics.
Directly or working with another employment specialist or therapist, conduct process groups utilizing cognitive behavioral health therapy and criminogenic thinking.
Directly or working with an employment specialist, case manager, or sector manager, identify a portfolio of employers appropriate for individuals receiving numerous services or individuals who have special needs.
Assist clients with Medicaid applications and renewals, as well as facilitating access to other medical, dental, and ancillary benefits.
In the form of one-on-one follow-ups or multi-disciplinary staffings, coordinate with other staff working with the client, while obtaining the necessary consents and maintaining confidentiality in accordance with HIPAA and when applicable, 42 CFR Part 2 or other relevant confidentially laws.
Document interactions, services, progress, and milestones in the necessary electronic client systems within expected time-frames and in desired manner
Office administrative duties for satellite sites

Required Knowledge, Skills and Abilities:
Passion for social justice
Knowledge of the reentry process and cross-sectoral barriers faced by individuals upon release.
Knowledge of Medicaid in Illinois desired.
Excellent communications (oral and written), computational and interpersonal skills.
Computer literacy; able to use MS Office Suite (Word, Excel, Outlook) applications.
Self-reliant, resourceful and outcome-driven with the capability to function in a fast-paced, start-up work environment.
Previous experience documenting services in an electronic health record system or case management system desired.
At least two years’ experience performing clinical assessments and individual/group therapy highly desired, preferably within substance use provider, mental health center, or other social service organization.
At least two years’ experience providing services to individuals involved in the justice system.
Experience providing employment and/or training services to low-skilled, low-income adults highly desired.
Bachelor’s degree in related discipline required.
Master’s degree in counseling or social work highly desired.
Certified as a CADC desired or ability to obtain CADC within two years of hire and/or
Licensed, or eligible to be licensed, as a professional counselor or social worker in the state of Illinois.
