QUALIFICATIONS: Requires knowledge of person centered, strength-based counseling, and recovery planning using ASAM criteria. Also requires good communication, writing, and organizational skills and a working knowledge of Microsoft Office. Must have CADC. Minimum of 12 months of continuous sobriety required for persons recovering from a substance use disorder. Submit to DHS Schedule L background checks and meet the eligibility requirements for employment. Experience in substance use disorder treatment, degree in a Human Services-related field, and Bilingual preferred.
RESPONSIBILITIES AND DUTIES:
Provide person-centered counseling and education services.
Conduct individual counseling sessions.
Facilitate group counseling sessions.
Develop and present educational materials utilizing individual assignments and lectures on a variety of recovery based topics including: substance use disorders, recovery planning, relapse prevention, living skills, and other relevant topics.
Provide activities to family members or significant others such as family conferences and organized “Family Nights.”
Use non-confrontational motivational strategies to promote engagement and retention in treatment, adherence to rules, and positive change.
Provide person-centered support, recovery coaching, and care coordination while carrying a caseload as needed.
Collaborate and assist individuals and their family/significant other(s) in developing their individual Recovery Plan and Continuing Recovery Plan.Be a resource to the individual in identifying the treatment methods (e.g. groups, assignments, etc.) and the other recovery support services to meet their Recovery Goals and objectives.
Provide care coordination needed for the individual and family. Be an informational resource and act as advocate to obtain support services during the course of treatment and following discharge from treatment. Including, but not limited to crisis intervention if warranted.
Provide crisis intervention services to the individual and family as needed.
Monitor the implementation of each individual’s Recovery Plan.
Collaborate with the individual and others involved in his/her recovery to evaluate treatment effectiveness and update the Recovery and Continuing Recovery Plans throughout the course of treatment and make changes as needed.
Provide progress and other reports to those involved in the individual’s treatment (e.g. DCFS, DHS, TASC, courts, probation, employers, etc.)
Coordinate care with outside agencies to provide other services as needed by the individual.
Provide utilization review updates to insurance or other managed care providers.
Complete all documentation in a timely manner
Referrals for Nursing, Mental Health Professional, or other services
Staff service reports
Other documentation as required
Meetings, In-Services, Conferences and Committees
Attend and/or present in-services or workshops as scheduled.
Attend and/or present staff meetings as scheduled.
Participate in various organizational committees as requested.
Make medications available for resident self-administration, monitor supply of medications, and notify Nurse or Director of need for refills.
Provide programmatic/treatment coverage and services and carry additional caseload when necessary (e.g. vacation, illnesses, personal emergencies, etc.)
Provide transportation as necessary.
Supervise clients in various activities.
Perform urine and breathalyzer tests according to procedures.
Comply with all applicable Client Accounts procedures.
All other duties as assigned by supervisor.
Counselors in Extended Care only: Notify Dietary Building Services Supervisor when dietary service worker fails to show for lunch or dinner meal; prepare and serve meal according to “Lunch & Dinner – Instructional Sheet.”
