Family Guidance Centers, Inc. (FGC), a not-for-profit behavioral healthcare organization, treats and prevents substance abuse, as well as an array of other related problems. Essential Duties and Responsibilities: Responsible for providing patient financial assessments, DUI evaluations, DUI updates, Investigative Reports and the facilitation of Driver’s Remedial Education groups. Serve as primary counselor to patients admitted to the OMT program. Will carry a small OMT caseload in addition to DUI responsibilities. Supports the Mission, Values, and Vision of Family Guidance Centers and the facility.
Discern and assess the patient’s needs in order to determine the type of evaluation needed.
Complete financial assessment of the patient.
Administer the Driver’s Risk Inventory (DRI) questionnaire to patient.
Input the DRI data into the computer and generate the DRI results.
Prepares and submits required reports.
Review DRI results with the patient for discrepancies.
Complete the clinical interview.
Refer patient for urine toxicology test.
Complete a computer generated report summarizing the DRI results, the clinical interview and UA results within specified time frame.
Obtain all signatures as required.
Disseminate reports as required.
Participate in treatment team staffing as needed.
Document in the medical record per DASA and FGC regulations.
Obtain needed and appropriate referrals to ancillary agencies.
Facilitate Driver’s Remedial Education classes per DASA regulations.
Documents in medical record and maintains patient confidentiality in accordance with regulations.
Complete an individualized treatment plan for each patient on caseload, addressing all needs identified in the psychosocial assessment.
Provide group and individual sessions to patients in accordance with their treatment plan problems, goals, and objectives. In order to accommodate our patients’ needs, some evening hours are required on a rotational basis.
Properly document all services provided in individualized progress notes per DASA, Joint Commission, CARF, and FGC regulations, standards, and policies, summarizing the patient’s status and ongoing treatment needs.

Education and/or Experience: Bachelor’s degree in Psychology, Social Work or a closely related curriculum, from an accredited institution preferred. DUI experience preferred.
Certificates, Licenses, Registrations: A minimum IAODAPCA Certification at the CADC or CARS level is required. Required to attend one complete DUI orientation training session offered or approved by DHS/DASA.
