Job Title: Certified Family Partnership Professional/Mental Health Professional
Job Summary: The CFPP/MHP utilizes personal experiences to facilitate and support the care, encouragement and education of those caring for family members or loved ones who have been diagnosis with a mental health condition in McHenry County. The CFPP/MHP has primary responsibility for facilitating and supporting caregivers. The CFPP/MHP advises and assists the NAMI McHenry County Board in evaluating our programs of care and provides budgetary recommendations for program support and improvement. The CFPP/MHP serves as the NAMI McHenry County liaison to community stakeholders and other network agencies and effectively collaborates to design and deliver a system of care that meets the behavioral healthcare needs of McHenry County and its residents. As the leader in the recovery process and family care, the CFPP/MHP is aware of personal needs to maintain the wellness of their own mental health and actions to take for their loved one living with a mental health condition.
General Responsibilities of Certified Family Partnership Professional/Mental Health Professional:
Collaboration: Works with the McHenry County community, consumers and families, healthcare providers, human service organizations to assure their active participation in the recovery process.
Planning: Prepares and directs the planning processes that promote goal identification and evaluation. Key outcomes include:
Periodic review and evaluation of system services provided by NAMI McHenry County.
Help develop an annual written plan for services provided.
Provide pertinent program information that will assist in developing an annual budget.
Organizing: Consults with community stakeholders regarding the most efficient delivery of prevention, early intervention, habilitation, treatment and supportive services that are recovery and resiliency focused and that promote self-determination and choice.
Coordinating: Coordinate with other agencies to share the role of NAMI-MC within our system of care and our ability to support the recovery of their clients and support family members.
Professional Background and Experience: At a minimum the following is required:
Education: Hight School Diploma required.
Must have 5+ years of experience in a community human service setting.
Must have training in the recovery model and experience working with Boards.
Must be current in the certification or near completion of certification.
Must have a self-disclosed personal experience gained through caring for a loved one with a mental health diagnosis.
Skills & Capabilities:
The CFPP/MHP should have written skills and oral communication skills including the ability to give formal presentations in small and large groups.
Have a valid driver’s license.
Provide certificate of automobile insurance per policy.
Proficiency in Microsoft Office.
Must demonstrate understanding of Recovery Principles.
Ability to work both independently and in a team environment.
Ability to advocate for, motivate and empower others.
Preferred bi-lingual in Spanish.
Ability to read, write and communicate in English.
Ability to lift and/or move at least 10 pounds using proper body mechanics.
Visual acuity to read a computer screen and other documents.
Ability to utilize the telephone and respond to callers.
Work Hours: Part-time
Must be able to work flexible hours to meet consumer and program needs.
